Please fill out this form as completely and accurately as possible so we can proceed with your pet’s care.

Surgical Procedure Consent Form

Your pet will have an intravenous catheter placed before anesthesia. An intravenous catheter provides a direct route to the bloodstream. The use of intravenous fluids during surgery facilitates adequate cardiovascular performance, reduces stress on the heart, assists in maintaining your animal’s temperature, hydration, blood pressure, and reduces the time it takes for your animal to fully recover from anesthesia. While undergoing the procedure, your animal will receive anesthetic drugs that prevent pain. Additional pain medications will be provided, as needed, to control the level of your animal’s discomfort both after surgery and during their recovery at home.

We highly recommend your pet having preanesthetic bloodwork to evaluate major organ function and to rule out any underlying metabolic disease prior to anesthesia unless these screens were performed at your presurgical consult or within the past 3 months.

For mass removal procedures only: Biopsy/Histopathology

Would you like your pet's mass submitted to the laboratory for testing? A biopsy provides crucial information about the nature of the mass and will determine if it's cancerous or benign. Testing will also help provide a prognosis and guide a treatment plan if indicated.

I understand that some risks always exist with anesthesia and/or surgery and that I am encouraged to discuss any concerns I have about these risks with the attending doctor before the procedure(s) is/are initiated. I understand that during the procedure(s) or operation(s), unseen conditions may be revealed that necessitate additional treatment and/or intervention. I hereby authorize the performance of such life-sustaining procedure(s) as are necessary in the veterinarian's professional judgment and that I will be responsible for all costs associated with these procedures. I have been advised as to the nature of the procedures or operations and the risks involved, including the possibility of death. I realize that no guarantee can be made legally or ethically to me regarding the outcome of any procedure performed.
